Carpal tunnel syndrome (腕管综合征) is pressure on the median nerve as it passes through the carpal tunnel on the palm side of the wrist. What carpal tunnel is

Numbness, tingling, or pain in the palm-side fingers often mean pressure on the median nerve in the wrist. That narrow passage is the carpal tunnel (腕管综合征).

It is not the same as mommy wrist (De Quervain’s). Mommy wrist sits on the thumb-side tendon sheath. CTS sits on the palm side, where the median nerve is compressed.

02 · Loads

What loads the tunnel

Anything that raises pressure inside the carpal tunnel can irritate the median nerve. Several factors often sit together.

Anatomical factorsA smaller tunnel or prominent bone. Some wrists start with less room for the nerve and tendons. Symptoms can appear with less load than in a wider tunnel.

03 · Signs

What the hand often feels like

Sensory change usually follows the median-nerve map. A physician confirms; these signs are a pattern, not a diagnosis.

1. Thumb, index, and middle fingers

Pain, numbness, or tingling in the thumb, index, and middle fingers, sometimes the ring finger on the thumb side.

2. Night flare

Symptoms often wake people at night or first thing in the morning, when fluid pools and the wrist stays bent.

3. Grip and fine work

Weaker grip, dropping objects, or clumsy fine tasks when the nerve stays under pressure.

04 · Screen

The Phalen screen

This short check is the Phalen screen. Numbness or tingling in the median-nerve fingers during the hold is a positive screen. It is not a diagnosis; a physician confirms.

1. Flex both wrists

Bring the backs of the hands together so both wrists bend fully toward the palms (a prayer-in-reverse position).

2. Hold about one minute

Keep the wrists flexed. Stop sooner if tingling becomes sharp.

3. Note the fingers

Numbness or tingling in the thumb, index, or middle fingers during the hold is a positive screen, not a diagnosis.

When to seek care first

These signs sit outside a typical desk-strain picture. Get assessed the same day before home presses or elective TCM visits. PULSE physicians refer when imaging, nerve studies, or surgical review are the safer next step; TCM complements orthopaedics and physiotherapy.

1. Severe pain

Sudden or severe wrist or arm pain that does not ease with rest.

2. Increasing weakness

Growing weakness in the hand or arm, or dropping objects more often.

3. Spreading numbness

Numbness that spreads quickly or leaves the hand clumsy through the day.
