TCM for gout

A sudden hot, swollen big toe at night often means a gout flare. Acupuncture, herbs, purine-aware eating, and barley or mulberry-twig tea usually help between visits.

In brief

A sudden hot, swollen big toe at night that peaks within hours often reads as gout (痛风), a crystal arthritis that affects more men than women and often flares around festive meals. About 90% of attacks start in the first big toe; elbows, wrists, fingers, knees, and ankles also flare. Uric acid from purines stacks past about 6.8 mg/dL before crystals form. TCM reads damp-heat (湿热). Clinic care uses acupuncture and individualised herbs. Between flares: limit purines, drink about 2 L of water, and cool barley tea or mulberry-twig tea.

01 · Signs

Symptoms of gout

A sudden hot, swollen big toe at night often reads as gout (痛风), a crystal arthritis that affects more men than women. In Singapore it often flares around festive meals such as New Year. About 90% of attacks start in the large joint of the first big toe. Elbows, wrists, fingers, knees, and ankles also flare.

1. Sudden, intense joint pain

Pain is most severe within the first 4–12 hours.

2. First big toe, or another joint

It can affect any joint. The first metatarsophalangeal joint is the usual start. Elbows, wrists, fingers, knees, and ankles are the other common sites.

3. Redness, burning, swelling, tenderness

The affected joints look and feel hot.

4. Limited range of motion

The joint will not move through its usual range while it is swollen.

5. Night start, days of flare

Attacks typically begin at night and can last 3–10 days.

Gout or an infection

Urgent Western care

A hot swollen joint with fever can be septic arthritis, not a gout flare. Kitchen drinks do not belong in that picture.

Seek urgent care

Fever with a hot swollen joint, sudden inability to walk, or a deformed joint needs urgent Western care. Recurrent flares without fever still warrant a GP uric-acid review alongside TCM.

Gout is caused by a build-up of urate crystals in your joints. Those crystals form when uric acid in the blood is high.

Uric acid is like sugar in tea: it dissolves, until you add too much. Then it stays as crystals no matter how much you stir. The crystals settle in joints and tendons.

What is uric acid?

Uric acid is a waste made in the body and when you digest purine-rich food. It usually stays dissolved in the blood and leaves in urine via the kidneys.

  • The kidneys do not remove the excess uric acid efficiently
  • You consume too much food that is rich in purines
  • The body produces too much uric acid

HyperuricaemiaA high reading, often silent. Blood uric acid above 6.8 mg/dL. You may have no symptoms yet. Not everyone with hyperuricaemia develops gout.
First gout attackCrystals reach a joint. Uric acid becomes concentrated enough that crystals form in a joint. The first attack usually lands in a single joint.
Recurrent / chronic goutFlares lengthen and crowd. After the first attack, flares may last longer and hurt more. The gap between flares may shorten. More joints may flare at once. Joint damage may be seen.

04 · Risk

Risk factors for gout

Flares are intense. Diet, drink, weight, and kidney function all change how often they return.

1. Age and male sex

Risk rises with age. Gout is more common in men.

2. Purine-rich food

Internal organs, some seafood, red meat, soybean products, and some vegetables high in purine add to the uric-acid load.

3. Alcohol and sweet drinks

Too much alcohol, beer, and artificially sweetened drinks raise risk.

4. Weight and high uric acid

Obesity and a high blood uric acid sit together with flares.

5. Kidney function

Chronic kidney disease, or poor kidney function, means less uric acid leaves in urine.

6. Family history

Gout can run in families.

Chinese medicine treatments help reduce damp-heat, condition the body, and make recurrent attacks less likely. They sit beside GP or rheumatology urate-lowering care. They do not replace it.

What can I do to avoid gout?

A willingness to change diet and lifestyle is the most important factor in preventing future attacks. In Singapore, steamboat organs, shellfish, beer, and sweet drinks are a common festive load.

Everyday habits between flares

Water, drink choices, and kind movement sit with the purine list. They do not replace a uric-acid plan from your GP.

1. Drink more water

Water helps the kidneys eliminate uric acid. At least 2 litres a day for a typical adult, unless a doctor limits fluids.

2. Restrict alcohol and beer

Alcohol raises gout risk. Cut back, especially beer, around festive meals.

3. Restrict artificially sweetened drinks

Artificially sweetened drinks sit with high uric acid. Swap for water.

4. Exercise regularly, not too hard

Vigorous exercise can build lactic acid and raise the internal acid load. Keep a regular, moderate habit.

5. Comfortable shoes

A tight shoe on a hot first-toe joint adds load. Wear comfortable shoes. Keep a healthy lifestyle overall.

Can I drink mulberry twig tea in pregnancy?

No. Mulberry twig tea (桑枝茶) is used to move blood through the joints. Skip it in pregnancy. Steep 10 grams for 10 minutes, two or three times a week, only when you are not pregnant.

How can I prevent gout flares?

Limit purine-rich food, drink at least 2 litres of water a day unless a doctor limits fluids, restrict alcohol and sweet drinks, exercise regularly without going so hard that lactic acid builds, wear comfortable shoes, and keep a healthy weight. Clinic acupuncture and herbs address damp-heat so flares are less likely to return.
