An itchy rash that keeps flaring is eczema (湿疹, also called atopic dermatitis, 异位性皮炎). It is among the most common skin conditions in Singapore, and occurrence here is among the highest in the world.

About one in five school-age children and teenagers, and about one in ten adults, live with atopic dermatitis. About a quarter of those adults have a moderate-to-severe form.

Modern medicine names several factors behind eczema. Genes play an important role, as eczema often runs in families. Skin-barrier defects, immune dysfunction, and environmental loads sit alongside that inherited lean.

Wind-Dampness-Heat from the Spleen and Stomach

TCM reads eczema as a Wind-Dampness-Heat (风湿热) constitution. People with this irritable lean often over-consume spicy, pungent, or oily foods, which weaken the Spleen and Stomach.

The Spleen and Stomach govern digestion and the conversion of food and drink into the essence that nourishes the body. When they fail to convert that load fully, Dampness-Heat arises. With Wind invading from outside, and Dampness-Heat accumulating beneath the skin, the two forces combine as an itchy rash.

03 · Stages

Stages of eczema

Eczema can be read in three stages: acute, subacute, and chronic. The stages describe the severity of the skin lesions rather than a one-way timeline. A rash may start at the subacute stage and move to acute. The same patch may cycle through the stages many times, and it may start or stop at nearly any stage.

The main principle is to clear heat, eliminate dampness, and relieve the itch. Herbs, named topicals, and acupuncture are matched to the stage after pulse and tongue.

05 · Skin care

Skin-care tips for eczema

Daily habits sit beside clinic care. Scratching, hot water, and harsh soap all feed the next flare.

1. Do not scratch, rub, or pick

Scratching aggravates the eczema and can lead to pigmentation, open wounds, infection, and scars.

2. Skip harsh soaps and detergents

Use gentle soaps or soap substitutes instead of ordinary detergents that strip the barrier.

3. Do not wash with hot water

Water hotter than 40°C makes moisture evaporate faster after the shower and increases transepidermal water loss.

4. Moisturise often

Non-steroid moisturisers do not thin the skin. Apply them liberally through the day, including after swimming.

5. Keep the barrier in dry or cold air

In a dry or cold environment, keep the skin well moisturised so the eczema is less likely to flare.

6. Choose cotton over wool or linen

Wool and linen tend to irritate. Cotton is usually more comfortable on sensitive skin.

7. Ease off foods that feed the flare

Heat, damp, and toxic-heat loads can worsen the itch. Spicy food, seafood, beef, mutton, and spices such as coriander, chives, celery, ginger, onion, and garlic are common flare foods.

Topical steroids

Topical steroids can settle a flare. They should not be used without supervision. Ask your doctor before starting or stopping them, and do not stop suddenly.

These sit outside a typical flare. Seek conventional medical care the same day.

1. Spreading infection

Spreading redness, heat, pus, or fever after scratching or an open wound.

2. Sudden widespread rash

A rash that spreads quickly over large areas of skin.

3. Severe allergic reaction

Swelling of the face or throat, trouble breathing, or a sudden systemic reaction.

How long does it take for TCM to work for eczema?

TCM focuses on the internal imbalance rather than only calming the surface, so timelines vary with severity. For acute flare-ups, itching and redness may ease within 1 to 2 weeks. For chronic eczema, repairing the skin barrier and restoring Spleen and Stomach function typically needs 3 to 6 months of consistent herbal medicine and acupuncture.

Can I combine TCM treatments with my prescribed steroid creams?

Yes. Integrative care is common. You do not need to stop Western medicines or topical steroids abruptly, as a sudden stop can trigger a rebound flare. Leave a two-hour gap between oral Western medicines and TCM herbal prescriptions. Tell your TCM physician about any steroid creams or antihistamines you are using.

Is acupuncture painful, and how does it help my eczema?

Acupuncture for eczema is generally well tolerated. Physicians use sterile, hair-thin needles; most people feel a mild aching or tingling (De Qi). Needling is used to move qi and blood, clear trapped heat, drain dampness, and ease severe itching.

What foods should I avoid during an eczema flare-up?

From a TCM view, eczema is driven by internal Dampness and Heat. Ease off heat-inducing foods (spicy food, deep-fried food, beef, and mutton), dampness-inducing foods (dairy, refined sugars, and cold or raw food), and common toxic-heat loads (shellfish such as crabs and prawns, alcohol, and heavy spices such as garlic and ginger).

Can TCM permanently cure my eczema?

Eczema (atopic dermatitis) is a chronic genetic condition, so there is no definitive cure in either Western or Chinese medicine. TCM is used to push eczema into longer remission by strengthening the Spleen, clearing Dampness-Heat, and naming your triggers, so flares are less frequent and less severe.

Are TCM herbal treatments safe for children with eczema?

Paediatric TCM is used for childhood eczema under a licensed physician. Because children’s digestive systems are more delicate, the physician prescribes a gentler, customised dose of herbal powders.
