TCM tips for a frozen shoulder

A shoulder that locks and hurts at night is 冻结肩, the fifty-year shoulder. Acupuncture, tuina, moxa, and gentle range work at home usually help.

In brief

A shoulder that locks, hurts at night, and makes combing or dressing hard is frozen shoulder (冻结肩, also 五十肩). Western medicine calls it adhesive capsulitis. Most common between 40 and 60, twice as common in women. Freezing, frozen, then thawing; thaw often takes 6 months to 2 years. TCM names Kidney-Liver deficiency or qi-blood stasis from wind-cold-damp (通则不痛,不通则痛). Clinic care matches the stage; needling often uses Jian Yu (LI15), Jian Liao (TE14), and distal GB34. At home, press GB34 on the opposite knee and walk the fingers up a wall.

01 · What it is

What a frozen shoulder is

冻结肩 · 五十肩 · adhesive capsulitis

Combing hair, dressing, or reaching a high shelf becomes hard when the shoulder locks and hurts. That picture is 冻结肩, also called 五十肩, the fifty-year shoulder. Western medicine names it adhesive capsulitis: pain, stiffness, and a restricted range in the joint.

It often appears with no associated injury. It usually affects people between 40 and 60, and it is twice as common in women as in men.

The smooth tissues around the joint become inflamed, thicken, and tighten. Scar tissue develops over time and range of motion drops. A rotator cuff injury, tendinitis, or long-term immobility of the joint can lead to frozen shoulder too.

02 · Stages

Stages of a frozen shoulder

Frozen shoulder develops slowly through three stages. Each stage can last a couple of months.

Freezing, frozen, and thawing
Stage What you feel Typical window
FreezingPain builds first.
  • Pain with any movement, sometimes severe
  • Range of motion shrinks as the pain worsens
  • Worse at night, and sleep is often disturbed
A couple of months. Night pain is a common tell.
FrozenStiffness leads.
  • Pain may ease, but the shoulder becomes stiffer
  • Range of motion is very limited
  • Daily tasks stay hard
A couple of months of a locked, limited joint.
ThawingRange begins to return.
  • Motion improves gradually
  • Strength follows more slowly than range
A complete return to usual strength and motion typically takes from 6 months to 2 years.

Signs that need urgent care

These signs sit outside a typical frozen-shoulder picture. Get assessed the same day.

1. Cannot lift the arm after a fall

Sudden inability to move the arm after a fall, twist, or impact.

2. Deformed joint

The shoulder looks out of place or the outline has changed.

3. Hot swollen joint

A hot, red, swollen shoulder, especially with fever.

4. Spreading numbness

Numbness or weakness that spreads down the arm or into the hand.

03 · Pattern scan

TCM causes of a frozen shoulder

Kidney energy declines with age and cannot nourish bone as before. Liver stores blood and moves qi. Wind, cold, and damp in the shoulder meridians can also stall qi and blood. If there is free flow, there is no pain; if there is no free flow, there is pain (通则不痛,不通则痛).

Common TCM readings for a frozen shoulder
Pattern Typical signs Clinic lean
Kidney and Liver deficiency肝肾亏虚: yang qi weakens with age.
  • More common between 40 and 60
  • Kidney dominates bone; Liver stores blood and regulates volume
  • Collaterals of the shoulder are under-nourished
Needling, herbs, and warmth to nourish and move what the channels can no longer carry.
Stagnation of qi and blood气滞血瘀: wind, cold, and damp stall the meridians.
  • Long exposure to air-conditioning, wind, or icy drinks
  • Fixed shoulder pain that can lock into frozen shoulder
  • No free flow, then pain (不通则痛)
Needling, cupping, tuina, and moxa to move qi and blood and warm Cold in the channels.

04 · Clinic methods

TCM treatments for a frozen shoulder

A physician matches methods to the pattern and to whether the shoulder is still freezing, already frozen, or beginning to thaw.

Clinic lean by stage
Stage What the joint is doing Methods that often sit here
FreezingPain leads; range is shrinking. Night pain and a joint that protests every reach. Needling and moxa to ease pain and warm Cold in the channels. Gentle range only.
FrozenStiffness leads; pain may ease. Daily tasks stay hard even if the ache is quieter. Tuina and cupping to release tight fascia, with needling for range.
ThawingMotion is returning. Range improves; strength follows more slowly. Tuina and daily finger-walk sit together. Herbs when Kidney-Liver deficiency is named.
Clinic methods for a frozen shoulder
Method What it does for the shoulder When it sits in the plan
AcupunctureNeedling at selected points. Eases pain and supports range of motion in the joint. Often first-line in freezing and frozen, for both deficiency and stasis readings.
CuppingSuction over tight muscle and fascia. Improves circulation and releases tight tissue around the shoulder. When the joint is frozen and fascia is holding.
Tuina massageHands-on work on points and meridians. Relaxes the muscles around the joint and moves blood so stiffness can ease. Once pain is less angry, through frozen and thawing.
MoxibustionMugwort warmth on the channels. Warms and invigorates channels when Cold sits with the stiffness. Especially in freezing, when night pain and Cold sit together.
Chinese herbal medicinePowder or pill formulas after pulse and tongue. Du Huo Ji Sheng Tang is often used for joint and muscle pain: it invigorates blood, dispels wind, and eases pain. Gui Zhi Tang is used when restricted movement sits with Wind-Cold-Dampness. When Kidney-Liver deficiency or Wind-Cold-Damp is named; not a fixed shoulder pack.

Jian Yu, Jian Liao, and Yang Ling Quan

When to book for a frozen shoulder

If night pain, a shrinking range, or a locked shoulder has lasted weeks, a physician can name whether it reads as Kidney-Liver deficiency or qi-blood stasis and decide which methods belong in your plan.

05 · Home care

Acupoint massage and stretches for a frozen shoulder

Yang Ling Quan (GB34) is pressed on the knee opposite the stiff shoulder. The fingers, not the shoulder muscles, walk the wall. Stay inside a kind range.

Finger walk, comb, back, and cross-body

Walk the fingertips up a wall from waist height, 10–20 times. Comb-the-hair raise, 10–20 times. Interlace fingers behind the back and lift with the good arm. Cross-body hold 10–15 seconds, 10–20 times.

The fingers, not the shoulder muscles, walk the wall.

06 · Daily habits

Home habits while the shoulder settles

Wind, cold, and damp stall qi and blood in the shoulder meridians. Kind range work sits alongside that, not instead of it.

Everyday loads that stall the shoulder
Habit What it does What to try
Air-conditioning and windLong exposure on the neck and shoulder. Wind-cold-damp can stall qi and blood in the meridians. Cover the shoulder in cold rooms. Do not sit under a vent.
Icy drinksDaily cold load. Cold in the channels is a common frozen-shoulder reading. Swap iced drinks for room-temperature or warm fluids.
Kind rangeFinger walk and the three stretches. Forced shrugs from the shoulder muscles can aggravate a freezing joint. The fingers walk the wall. Stop if the stretch turns sharp.

FAQs

Frozen shoulder questions

Common questions on 冻结肩, stages, clinic points, GB34, and home massage.

What is frozen shoulder in Chinese?

Frozen shoulder is 冻结肩. It is also called 五十肩, the “fifty-year shoulder,” because it is most common around middle age. In Western terms it is adhesive capsulitis: the shoulder capsule thickens and tightens so reach, combing, and dressing become hard.

Can acupuncture help frozen shoulder?

Acupuncture is commonly used for frozen shoulder. Fine needles at selected points are used to ease pain and support range of motion. A physician matches points to whether the shoulder reads as Kidney and Liver deficiency or qi and blood stasis, often alongside tuina, cupping, moxibustion, or herbs.

What are the three stages of frozen shoulder?

Freezing, frozen, and thawing. Freezing brings rising pain, often worse at night, and a shrinking range. Frozen is stiffer even if pain eases, and daily tasks stay hard. Thawing is when range returns; a full return of strength and motion often takes 6 months to 2 years. Each stage can last a few months.

What acupressure points help frozen shoulder?

Yang Ling Quan (GB34) is the home point taught here. It sits in a hollow in front of and below the head of the fibula, on the outer knee. Press the knee opposite the stiff shoulder, make small circles, and gently abduct the arm.

Which acupuncture points are used for frozen shoulder?

Clinic needling often uses Jian Yu (LI15) and Jian Liao (TE14) on the shoulder, with distal Yang Ling Quan (GB34). The set is chosen after pulse and tongue, not as a fixed protocol. Home press stays GB34 on the opposite knee.

How long does frozen shoulder take to thaw?

Each stage can last a couple of months. A complete return to usual strength and motion typically takes from 6 months to 2 years. Clinic methods and daily range work sit inside that window; they do not set a fixed number of sessions.

Can I massage a frozen shoulder?

Gentle range work and contralateral GB34 press are the home habits on this page. Clinic tuina is used to relax the muscles around the joint and move qi and blood, after a physician names the pattern. Stay inside a kind range. Book if the shoulder stays locked, or if pain follows a fall.
